Case History

Health Status of the Visual System Evaluation

• External and internal examination (to include direct and / or indirect ophthalmoscopy).
• Neurological integrity (pupillary reflexes and extraocular muscle assessment versions).
• Biomicroscopy (when necessary).
• Gross visual fields.
• Tonometry. 

Refractive Status Evaluation

• Visual acuity – entering visual acuity with habitual Rx or unaided acuity (as indicated) and best corrected acuity.
• Subjective refraction and accommodative function.
• At least one of the following two tests:
• objective refraction by 1.Retinoscopy or 2. Keratometry.

Binocular Function

• Cover testing.
• NPC.
• Phorias  All testing (distance / near; horizontal / vertical) constitutes only one test category for binocular function.
• Stereopsis.
• Vergence testing.
• Fixation disparity.
• Prism reflex test.
